Without you, our patient, we have
no practice. We strive to make you our top priority
and treat you with respect, care, and concern. We treat
you as part of our team and involve you in the decision-making
process while creating your customized dental plan.
We encourage you to ask questions and make suggestions
at any point along the way.

A successful dental practice takes more than
a great staff and state-of-the-art equipment. A key ingredient
is enduring relationships with open communication and mutual
trust. It is also important that your results are comfortable,
esthetically pleasing, and affordable.
Our staff will work closely with you and listen carefully
to your concerns and desires. We will also educate you so
that you can make well-informed choices and decisions about
your treatment. Your comprehensive, individualized plan will
take time, effort, and collaboration. At times, we may need
to enlist the services of other specialists. We also ask that
you actively follow-up and participate in maintenance care
after your restorative plan is complete.
We look forward to meeting you! When you arrive for your first
visit, you will be asked to complete a health history form.
You will also be given a copy of our privacy policy and a
form to complete saying you received this. These forms can
be downloaded and completed before your visit. Please bring
a list of any medications you may take and the current dosages.
A staff member will review this information with you and assist
you with anything that may be confusing.
Our team will work with you to develop a comprehensive dental
plan that is affordable and meets your goals. If necessary,
a separate consultation appointment will be established with
Dr. Edwards to discuss the details of your plan.
In order to keep our costs and fees at a minimum for all patients,
it is expected that fees are to be paid at the time services
are rendered. If you are to undergo extensive treatment, we
will be happy to work out a financial plan that will be suitable
to everyone.
Dental insurance is intended to cover some, but not all, of
the cost of your dental care. Please bring your insurance plan
information with you on your first visit. At the end of each
visit, you will receive all of the information necessary to
file your insurance claim. We will work with you to assure
that you receive the maximum benefits to which you are entitled.
We will happily assist you in any way, or work with you to
find the answer to any questions you may have.
