Our patients are seen on an appointment basis.
We ask that you call in advance so that we may reserve a convenient
time for you. We strive to see all patients on time, and thank
you for extending us the same courtesy.
Our office is open Monday through Wednesday from 8am
to 5pm. We are open on Thursday from 7am to 2pm.
To schedule or change an appointment, please contact
us.
If you have an emergency, please contact
the office as early in the day as possible. We reserve time
for patients in this situation and we will try to respond
to your problem as quickly as possible.
First Visit
You will be required to complete the Patient Information
form upon your first visit. To save some time you may download
